Internal Santander ISA Transfer form

Does anyone know what the internal ISA transfer form is?
I have one Santander ISA, which I want to close and transfer the money into another Santander ISA.
I have seen this form below but it appears that it is for partial transfers and multiple ISA transfers. Is there another for one merging into another

Can't see why you shouldn't be able to use it for one-to-one transfer. However, you might get it done faster if you just call them.0

That's the right form. You only need to fill out one section each on the left and right.0
