I signed my DLA section saying they can contact GP/Psychiatrist, they did indeed write to the Psychiatrist. So am lost on how they want this. Plus mental health records are different to normal health records. Am not really happy about having to waste the Doc time to discuss my claim. Doc mean unwell or have a worry re health.
I realise it's kind of been answered already but 2 things "they" are two different organisations. By signing the DLA section you refer to ou've consented to the DWP contacting your GP/psychiatrist. You're now dealing with an independant organisation (albeit part of the same government) who will consider an appeal.
The form you've been sent is their standard form for obtaining further medical evidence. It is used if they want a report from a GP (or other medical practicioner) or if they are requesting your GP records, or, in your case, if someone is being sent out to perform a medical in your home. Once the medical is completed basically the doctor who comes to your house cannot give his findings to the tribunals service without your consent, that is why they've sent the form to you.Bought, not Brought0 -

Medical assessments can take up to 12 weeks (rarely longer) and then when the tribunal service receive the report it will be sent to you and the DLA department, the DLA department (or you) can challenge the contents and then the tribunal hearing will be booked.0
