Clearly someone has managed to access your Halifax account. So someone has your username, password and the other security question which you enter 3 from x.
So clearly its either your computer or someone you know that has done it.

    yup,
given that the access requires random 3 digits from one of the memorable/security info as well as the main password and the main user ID
Someone clearly has access to the WHOLE lot of data about accessing the account..as well as being able to almost pass mobile phone security questions to attempt to execute a divert request.

Sad to say...and not for the first time one must include in the list of suspects those who are 'nearest' to the OP in terms of friends and family who know a lot about the OP.

I contacted Halifax straight away, they said someone had transferred all of my ISA a few grand to my main account and was then trying to setup a new payee, presumably to clear my account out.

Once you get to the bottom of how this happened, make sure that you don't permanently lose the tax-free status of your cash

If you're really worried about changing passwords on your computer, why dont you ask if you can use the bank's computer to do it? If your local bank is anything like the ones near me, there are some rooms in which customers are taken when they need to discuss accounts etc. with a bank manager and there's usually a computer in these rooms.

They might let you borrow it.

Also, the guy who said change your password on an iPhone/iPad has the right idea :)

    Other tips:

    - Make sure your WiFi network is secured with a password.
    - If you use a wireless keyboard make sure it's encrypted.
    - Regular virus/spyware/malware scans.
    - Firewall.
    - Use a mix of upper/lower case letters, numbers and symbols (if possible) in your password.
    What will your verse be?

OP - Ask Halifax to return the cash to your ISA ASAP.
