My BIG (3L) diesel gets 40mpg quite easily, if I plan my journeys and drive carefully I get easily 50 mpg

Seems little point stating MPG without giving any context.
There is a 90%+ chance the OP's car is less than 3L... as a rough guess.

If 45p/mile works for a 3L Diesel then it should be perfectly reasonable for a 1.9D or 1.4 petrol.... (which are far more common)... BUT not (perhaps) for Hintza in his 2.8L Cherokee.

Have a search for the This is Money, True cost of running a car calculator. For a quick comparison, I put in the details of the car I am running privately, even trade price and no finance, it came out at 54ppm (and that didn't even include my snow tyres!).

Obviously its a good idea to use this or your own....
However in calculating your costs for work or the OP's situation you need to remember to use it twice...

If you don't and it comes out over 45p/mile you're likely to come away thinking you are being cheated when you're not.

The 45p/mile is meant to cover occasional business use* (under 10k/yr). That is you have a car that is taxed, insured and used.

Using it for business/OP's use will add....
    1) More depreciation in milage
    2) More frequent servicing
    3) More frequent replaceable items (brakes, tyres and clutch)
    4) Additional premium on insurance**

So if you usually do 5,000 /yr non business and do an additional milage calculate first on non-business and again with business to add the extras ...

    * If you do mainly business or you wouldn't have a car otherwise then you are meant to be better with a company car.... hence at 10k this switches to a lower rate....

**Doesn't apply to picking up sisters kids

this is your sister, just ask her to chuck in £20 a week petrol, simple!

and if you really want to know the exact price your paying for petrol, fill it up the beginning of the week, use it, fill it up at the end and see how much you had to fill it up by

    come on, common sense

The HMRC 45ppm has nothing to do with business or personal mileage, vehicles don't know which is which. It is a long ago set average figure for the whole costs of running an average car. Most decent cars in the real world cost a whole lot more and HMRC acknowledge this, simply staing that figure can (and is) exceeded, but that's all they'll allow for tax purposes.
Why not get her to phone around the local taxi firms for a long term deal for this. Absolutely certain it wont be as cheap as you.

Has your sister not got a nearer school? If not, has she enquired about help with transporting the kids or a school bus?
