T-Mobile micro-sim activation

Got a micro-sim from the tmobile shop to use in my new nexus 4, the bloke in the shop said I have to go online to activate it but the sites asking for a 14 digit sim serial number but the one they gave me only has a 12 digit serial number.
any ideas??
It should be on the paper that the sim was attached to0 -
There should also be two numbers in bracket, i called cs and they sorted it for me, its best to call from a landline if you can coz they disable your old sim and activate the new one.0
