Lloyds TSB mini cash isa - Re-apply ?

Have just got my Cash ISA statment.
Completely forgot to invest last tax yr, so the last time i put anything in was in 2005/2006 tax yr. This yr putting in max of 3K, but just looked at the end of the statement in tiny print and it says "due to HM customs rules, if i haven't paid any money in since april 2006, need to complete new application". Is this correct ? whats involved ?
I am guessing that this is all changed as used to be max investment of 9K, but now it says max investment 21 K.

    Yes it applies to all ISAs. If you miss a year it has to be treated as a new application. So you have to do a new application form.
    It's nothing to do with any change from 9 to 21K
