Chargeback Issue with Co-op

I hope somebody out there is able to advise me on this one because i'm totally stuck and don't know if what the co-op bank tell me is true.
I purchased some jewellry on 1st December as a Christmas gift but it did not arrive until after Christmas. Just before Christmas I contacted the seller who agreed to refund me if it did not arrive in time and I returned the item.
Since new year I have twice been promised refunds from the seller but she hasn't actually provided the refund yet and the goods have been returned.
I called co-op to enquire about a chargeback (visa credit card) but they informed me I must have an email from the seller stating that £12.99 was refund to card number 1234 5678 1234 5678 on xth January 2013.
The seller is now totally ignoring me and hasn't replied to my previous two messages. I don't expect there is anyway I can get her to send me such a formal email as is expected by the co-op.
I've had to do a chargeback with Capital Once Mastercard a few years back and had none of these problems. All I did was state the transaction amount and date and they sorted it out.
Can anybody advise how I can go about getting my money back, it's only £12.99 but I am beginning to feel increasing angry about the situation.

The Coop can only action a chargeback for non receipt of refund if you have proof that a refund has been offered/made. Without this they can't dispute it for you.0

I have an email from the seller stating that she is happy to refund me and another stating it will happen first thing on Monday morning (7th Jan) and nothing has happened.
I have printed these off to include with the letter I send to the co-op based on Martin's template.

Sorry, I should have been clearer.
The last email she sent me was on Saturday promising the refund on Monday 7th January first thing.
I emailed back late afternoon on Monday 7th Because I'd had no notification of a refund and since then i've heard nothing.0 -
Refunds can take a while to appear on your account. If she did it on Monday, it might take until next week to show, depends on how she processes the refund.0

That's what i'm hoping.
I could be being overly paranoid but they last message she sent was very passive aggressive and up until Saturday her communication had been prompt. Thus ignoring my last message is concerning me.0 -
Seems the co-op are very tight then.....
Not cost effective to action a chargeback that low (soory wont say the limit)
So in effect most banks would just refund as GOGW.
In effect for visa you need a refund reciept so...
We slow delivery jewellry co, have refunded card end 1234 the sum of £12.99 on the 1st of never.....
So you could send them a email asking if they have. Reply or not it could be tried.
Ask for the disputes department and call their bluff and say you now have this and see what they say... Dont waste your time with a letter.Never ASSUME anything its makes a>>> A55 of U & ME <<<0 -
Luckily she refunded me this morning and also sent the most passive aggressive, petty email I have ever received.
Still, the money is what I wanted, the childish rant accompanying it was just a bonus :j0 -
