Virgin Media Subject Access Request?!!?!?

Hi all, could use a little help here…

In July this year I entered into a payment plan with vm after falling behind with my bill after loosing my job. I specifically explained in the email correspondence when we were setting it up that I was using my house phone as a contact number for jobs. I found out in September that vm had put their voicemail service on my line in July when the agreement was set up and this had answered my phone before my desktop answer machine could take messages. After chasing around I found that there had been several messages left for me on this service offering interviews and there had been three messages from a recruitment agency in august chasing me to start a contract, when I didn’t answer these messages they gave the position to another candidate.

After being fobbed off by vm I took the case to cisas who are a complete waste of time!!! Virgin produced an account note that said I’d been told about the service and that a letter had been sent out, in my response I asked for a copy of this letter to be produced as I’d never received it and I asked the adjudicator to get virgin media to produce the notes that will have been made on my account during the 6 calls that occurred in September when I discovered the service. The adjudicator ignored this request and simply judged the case based on this one account note vm produced. Vm also claimed that after removing the voicemail service they could not leave messages on two occasions when they tried to contact me, I explained in my response that there had been a fault in the junction box in my area and this was the reason calls were not being directed to my house and that vm had been and repaired it on 1st October. Vm didn’t comment on this and the adjudicator simply referred to it as ‘my claiming there was a fault on the line’!?!?

Following the cisas decision I have spoken to vm technical services who happily emailed me the engineers notes, what I aim to do now is issue a subject access request asking for a copy or transcript of the call where vm claim I was told about the voicemail service, and also copies or transcripts of the calls that were made when I discovered the service was on my line and also all account notes that were made.

Does anyone know what address is best to send my subject access request to?
