    Sounds to me like OP might be the company secretary and has the job of completing the form for the first time for the company. OP may not have even driven it that day.

    I would give it to the boss, make him aware of his duty to keep accurate records, and that if he is wrong this could come back to bite him and let it be his problem.

    But the NIP wouldnt come in the OP's name if the vehicle was registered in the boss's name would it?

    Registered in the company name only then i would maybe expect it to be as you said.

    If we read the OP again there is more than one clue that OP is the company secretary, it doesn't say it is in his/her name just that it has come to them at work to identify the driver, it says the there are 3 possibles (does not say that one of those is necessarily the OP.) IMHO the NIP would then be sent to the home address of the chosen employee.

    Oh dear. So the OP may well be in trouble. Is it still the case the secretary is responsible?

    Is the boss or the secretary responsible for the lack of driver records also?
