You need to clarify the problem, as needing a correct "main policyholder" doesn't make sense.
There is a "main driver" (who you nominate when taking out the policy), and a "policyholder", not a "main policyholder".
As you are married, then the car is in your joint ownership, and so the name on the v5 is irrelevant.
(Does the policy stipulate anything about the policyholder being the Regustered Keeper?)

It seems they are unhappy with the main driver's profile for some reason.
You do need to sort this out as from their stance they look to be on the verge of cancelling the policy (and may not be paying the claim on the grounds they wouldn't have issued cover had they been told your missus was to be the main driver).0 -

It does look like that, but it is strange.
She was never involved in any accidents, she has owned cars in the past, with her name as the registered keeper on the V5, she does not do a lot of mileage, and does not use the car for work purposes.
On paper, she looks a safer/better person to insure than me.

I can't see why they are making a fuss about it. One of our cars I was the RK because I bought it but insurance was in my daughters name with me as a named driver. The insurance had to be done that way as she was the main driver.
Just tell them it was in husbands name because he was the one that went to the garage and signed everything for it but wife insured it because wife is the main driver.0 -
As an update.
The underwriters are now asking for a copy of the marriage certificate...
