I've moved this thread over to the family's board to see if you can get more advice.
I agree with other posters that the little one is too young to appreciate that it's his birthday so if his Mum want's it to be just the two of them then the way to make it special is to spend a lot of time with him, give him his favourite food and take pictures so that he will be reminded of what he did on his first birthday.
Otherwise, could she invite some friends with babies of a similar age around for a small party? Even three or four other mothers and babies or friends would be an event and wouldn't have to be costly with tea and cake....after all the babies won't care.

If anything at all she should include the older sibling. I can't imagine having a birthday celebration without him. But as others have said, the baby is too young to know or care. We just had a chocolate cake with a candle. Take photos because they'll be giggled at when he's older :-)0 -
My friends child was 1 in December. For both Christmas and birthday they just wrapped up a couple of existing toys and gave them. For our older daughters first birthday we had a family buffet style tea with a few friends and family, sandwhiches, sausage rolls, jelly etc and of course a cake. We did the same for her second too, then for her third we also invited the few little people she knew and did a few party bags, games etc...Her fourth is going to be slightly more of a challenge as she goes to nursery and though gets along with everyone doesn't have a set group of friends/a particularly close relationship with any of the other children, and the done thing seems to be initing the whole class. Although she lives in a flat is it big enough for a little tea party? Or how would you feel about having a few of them round your house (and that could be your present)?!0

I guess it depends on the size of the house but we went to an indoor teddy bears picnic for a friends 1st birthday.
Her Mum cleared the kitchen of furniture and put lots of little rugs on the floor. She used old wallpaper and drew a garden theme on the back this was then stuck all around the kitchen cupboards.
We had a lovely time, sang some songs, played pass the parcel - the children were to young really but they loved opening presents.DFW Nerd #awaiting number - Proud to be dealing with my debts!
