Help with questions on the PPI reclaiming questionnaire

I'm beginning the process of attempting to reclaim my PPI.

I applied for the card in the year 2000 give or take a year (I'm at work so don't have the exact date to hand). While sorting through some paperwork I found a print-screen of one of the screens during the credit card application for the Egg card and there's no mention of PPI on there.

I've stopped the PPI on what was my Egg card, it's now a Barclaycard. I stopped the PPI last week.

I'm in the middle of filling in the PPI-consumer questionnaire from this website and I have a few questions.

Question A.3 what's the name of the financial business you're complaining about?
Should I put Egg or Barclaycard?

Question A.4 what's the policy number of the payment protection insurance you're complaining about?
Is there a separate policy number and where would I find this?

Question B.1 when did you take out this payment protection insurance?
Is this the date I applied for the credit card as that's when the PPI started from?

Question B.7 have you ever made a claim on the payment protection insurance you're complaining about?
While I don't recall ever making a claim on this insurance (i'm sure I'd know) I did contact Egg back in 2005 or
2006 as there were transactions on the account that I hadn't made. Eventually Egg recognised that I hadn't made the transactions and refunded the money and issued me with a new card. Does this count as a claim?

Question C.2 what was your reason for borrowing the money (or taking out the credit)?
I took the card out to make internet purchases which doesn't seem to fit any of the categories listed.

Question C.3 if you borrowed the money to pay off other debts, please tell us more about those debts?
I think I may have transferred a credit card balance from a barclaycard to the new egg card but can't be sure.
Is this important?

Question D.7 when you took out this insurance, did you or your partner have any health problems?
Does this question relate to the insurance that I may have had in response to Question D.6?

Is it up to me to know what the card numbers were in the years since my first Egg card right up to where Barclaycard took over and issued me a replacement card?

    I'd put Egg, now owned by Barclaycard. It was Egg who mis-sold you the Insurance, so if it can only be one put Egg.

    With a credit card, it should be on your monthly statement. If it's not, then you obviously have quite a bit more research to do before you send this form off.

    Be sure this was the case, you didn't take it out at a later date did you?

    Put"No".
    Querying fraudulent transactions on your card is not claiming on your payment protection!

    Just put "general leisure pursuits" or some such.

    No, this is for loans. It's asking if you've consolidated your debt into further loans which may also have PPI and would perhaps need investigating as part of your complaint. Leave it blank, or put "Not applicable"

    Since you haven't detailed what you've answered at D.6, how can we know?

    Yes, if you don't have the account numbers you need to write a Subject Access Request (SAR) letter to the Bank. This should give you all information they hold on you for at least six years. It costs £10 and they have 40 days to respond.
    Do remember to mention that this was Egg, not Barclaycard.

    So my SAR request about my Egg card do I send that to Barclaycard or to the Egg Derby address?

    Send it to EGG and mark clearly on the envelope SAR REQUEST.

    Thanks Moneyineptitude I have one last question that's bothering me. At the time I took the card out I worked for Adecco on a long term placement at a company. Could it be claimed that I would have needed the PPI as I wasn't strictly a permanent employee in the normal sense. Way before I took the card out I took out some critical ilness cover which is still running so that might negate any concerns over my employment status at the time.

    I doubt the Bank will know the temporary nature of your employment at the time unless you actually tell them!;)

    Maybe to you it does.
    You certainly don't have to draw attention to the fact that it was temporary work.

    Thanks for your help so far.

    The situation now is that I've sent off the £10 cheque with the SAR request and had statements going back to 2007 so only 5 years. The card is still active and I stopped the PPI in June or July this year.

    I've seen other people on these forums get statements back from Barclaycard/Egg going back earlier, should I expect to get the same back too and should they have sent these back with the original SAR from me to them?

    By the way 2 days before I received the statements I received a photocopy of my SAR form and a letter from Barclaycard stating that they had not received a cheque with my request even though both were in the same envelope.
