Accident in supermarket lift - reporting it..?

If this is the wrong place, very sorry was unsure where to ask...

Unusual question but after trawling the internet i'm unsure who to contact. We had an incident with a lift on saturday afternoon, it closed on my sons head as the sensor did not pick up that a person was behind me (his dad was behind him) so we managed to stop it closing completely, however when the store checked the lift they did find that it only had certain sensors and below a certain height it thought nothing was there and closed. I want to report this to ensure a more serious injury is not caused or that this happens again to somebody else - is it HSE????
I need to start saving so I plan to save £2 a week to start with:beer:

    Contact Environmental Health at your local council - I think it's fairly likely to be one that they would deal with but if it isn't they'll be able to refer it to the HSE.
    Common sense?...There's nothing common about sense!

    The enforcing authority for a supermarket is likely to be the local environmental health department. The HSE tends to deal with higher risk workplaces such as factories and warehouses. However, the HSE can be contacted with any queries / concerns and should be able to help.

    Make sure that the incident has been put in the accident book at the supermarket. This will help to safeguard you, should your son have any latent injuries.

    that was the problem, it wasn't, no incident form or anyting. i have contacted their head office - awaiting to hear

    When we had an incident at our local supermarket we wrote to head office, and they told us that we needed to go back to the shop and get the incident recorded in the accident book. You'll probably need to ask at customer services for it. It should've been offered at the time.

    correct me if im wrong... but even IF the lift did close on him, dont they close gently and open once they touch anything (be it a bag or a head)?

    There must be sensors all the way down the doors on some lifts because I once saw a little boy doing this deliberately - standing with just his head sticking into the lift doorway so that the doors touched his head and then opened again. When I noticed him the first time, I rushed over to help him but by the time I reached him, he was giggling fit to burst and sticking his head back in the opening, waiting for the doors to do it again!
