    I have found out the form my hubby filled out was FP92A which we got from his doctors surgery. His GP had to fill it in and send it off.
  • I have an exemption card too, not related to income at all but as Limana68 describes, the doctor fills it in and you just send it off - the doctor's surgery should have one of the forms if you ask one of the receptionists. I qualified for it because my disability means I can't get out on my own any more.
