Dumb question about Santander ISA transfer form

Hi all,

I'm transferring a whole bunch of ISAs over to Santander and I have a question about their form.

In the part where they ask you to fill in the account number of the existing ISA you're transferring, there are enough boxes to contain both the account number and the sort code. But they only ask for the account number. I've already told them the bank and the address, but the sort code isn't explicitly asked for anywhere - screenshot here.

However, in the section above where I fill in my details they ask for "the account number and sort code of your existing Santander ISA", in that order, and then present the same number of boxes but pre-fill the first 4 digits of the sort code (which means they really want the sort code and then the account number ;) ).

So what should I put in for the ISA I'm transferring:

Just the account code and leave 6 boxes empty?
Account code and sort code?
Sort code and account code?
