PPI Questionnaire: Difficulty Answering Questions - Help!

Hello all,
I'm new to this, but am currently trying to get PPI back on a number of credit cards. I have written to the providers and asked them to send me copies of my agreements, but they are being quite difficult in not providing all of the details I have requested.
I am now about to undertake the task that is filling in the consumer questionnaire so that I can make my complaint official, but I’ve noticed a question that I'm unable to answer, which I'm thinking will make it difficult to pursue my claim.
The problem is that in the questionnaire, you are asked to provide details of how, where and when you took out each credit card agreement. But because I have had six cards in total over the last few years, I cannot actually remember where and when (and by which process) I took out each agreement. This means that when I fill in the questionnaire, it will make it difficult for me to say that I felt pressured into taking the PPI by the salesman, or the box was pre-checked on the form I filled in, etc.
There is an option to say that you can’t remember, but I figure that this will weaken my claim.
I suppose I could actually ask each credit card company for this information, but I’m thinking it’s pretty unlikely that they’ll tell me!
Any comments on what I should do would be very much appreciated. Has anyone else had this problem?

I have been emboldened to post for the first time as having read umpteen pages of this thread and skimmed a few more I am still unsure how to proceed.
My potential PPI claims (or not) dates back to the early 1900s and possibly as late as 2003-ish.
During that period between student days, graduation and getting myself set-up in self-employment I no doubt took out PPI insurance on loans, a consolidated overdraft and possibly also a credit card.
All my banking activity relates to The Clydesdale Bank while I also had a very nasty Barclaycard Sense for a while that I may or not have had PPI on.
I can find no mention of PPI on the Barclaycard statements I have remaining from that credit card for 2003-04 (I cancelled the card a bit later). However there is some chance that it may initially have had PPI on the card that was cancelled by me prior to 2003.
As for The Clydesdale I have popped into my branch and called their PPI people and in both cases their records do not go back far enough, they say, to cover the period where I may have had PPI.
The branch have asked me to write in a complaint to their Customer Engagement department but I am not really sure what I am complaining about in terms of specific instances, dates and so on where I may or may not have had PPI insurance through the bank.
Reading the threads I can kind of see that a SAR may not go back far enough to help me at The Clydesdale so I wonder if there is anything else I can or should do?
I have had the same Clydesdale branch account since 1988 but I obviously do no hold any of the paperwork prior to 1998 when I reckon I was cajoled into taking PPI on occasions 'to make sure my credit applications were accepted'.
As for the Barclaycard, I do have some statements although I can see no mention of PPI on them which surprises me a bit. I can obviously contact Barclaycard to confirm whether or not I took PPI on that card initially....
As for the Clydesdale, any help would be greatly appreciated. I am guessing that too much time may have passed to be able to get any satisfaction from them without me holding paperwork, dates and details on the PPI I paid for?
