Vodafone refuse to remove default

Does anybody know how to go about having a default removed from ones credit file?

In short, I received a large phone bill that I agreed to pay off in three installments.

Original payment was due February
100 pound payment (1/3 of total) made March
200 pound payment June (after many letters from myself)
40 pound late payment charge added and disputed by myself
July-Paid late payment charge to prevent damage to credit rating
July- after receiving credit file realised that a default had been registered against me.

Gone to ADR, regulator useless
