The form is online but you need the unique web address (and code) given to you once you do the instore demo. Or that is how it worked in Hayes's in Bath.
I also received a £50 off my next Tempur mattress purchase voucher, which is valid until 27th May (20 days after the pillow promo ends).

I went to the Feather & Black store in London, the sales assistant seemed a bit standoffish (I think she thought I was just after the freebie) but went through the pitch anyway. Took about ten minutes. Got the flyer which has the web address for the form - I think it's a unique web address as when I went back to the page it wouldn't load a second time.

Lots of Bensons for Beds seem to be offering this - I went to my local one and they were happy to help - even though I explained I wanted the free pillow from the start.

They give it to you instore when you do the demonstration and then you enter the details on the webpage, they then send you the pillow.
