Obviously, you weren't following the advice (from April!) at all. The procedure is to complain to the firm who sold you the PPI in the first place. FOS can only become involved if the firm refuse to uphold your complaint. This complaints procedure is the same for all, regardless of whether or not it was 2001 or earlier.
You send the Ombudsman's Complaint questionnaire, but you don't complain via the Ombudsman in the first instance.
Great Universal PPI complaints should be directed to Shop Direct, Liverpool (Google the full postal address).

The person who sold the PPI was great universal in 2001, I tried calling them but they have no details for me that far back, I have now printed off the questionaire and will send first thing monday thanks. I am only just jumping on the bandwagon for the PPI after finding out that it had been put on an rbs card and then removed 2 yrs later by rbs without my knowledge, It is a whirlwind of phone numbers and different departments trying to get any details for great uni, so will have a look for the shop direct address now, many thanks. Oh I wasn't even aware that it was shop direct until I spoke to a member of the ombudsman team.

I would be aware that with it being 2001, the company can refuse to consider the complaint as it was pre-regulation. As you've said, FOS cannot then become involved.

Yes, my main concern was that the OP seemed to think that FOS handled complaints from the outset without them being sent to the original seller of the PPI.
This is indeed a pre-regulation complaint and FOS won't be able to adjudicate if the complaint is rejected.
