Cheers Mazza but managed to find one online free of charge and just filled in the gaps already to be signed when I can find a couple of idiots - sorry meant friends - to sign it tmoz and fingers crossed problem solved. Also rattled off a letter to my MP this after to have a rant about the fact that there seems to be no one clear and defined process in place either from the passport office or indeed from the several solicitors I have spoken to. Not something I'd normally do but just got so frustrated with the whole damn thing! Still Im sure I'll get a very polite fob off if nothing else...0

I'm not surprised you're frustrated. The passport office are stipulating that they need 'proof'. The situation is a little unusual and not catered for particularly well with our existing methods of documenting name changes, e.g. deed poll/marriage certificate.
If the passport office are the ones that need the proof, then logic dictates that they will be able to tell you exactly what that proof should entail. If they can't, they're merely saying to you 'We need you to do something but we can't tell you what that something is'.

I'm joining this a little late as I can see you have resolved it now. I had a very similar situation, I was born with one surname then my mum 'changed' it to my stepfathers name when I was very little - without my 'permission' or knowledge and something I would never have agreed to!
When it came time for me to get a passport (although this was some years ago now:o) I had to get my GP to sign the back of my photo and sign a letter to say that he had known me as *this* name for *this* long. When I married, as another poster has said, my marriage certificate says *stepfathers surname* formally known as *birth name*.
I'm having a :mad: time with the bank just now trying to change my name back to my birth name after being divorced for two years. Not because it's particularly difficult, I have the paper trail, it's just them making constant c*ck ups!
I hope the deed poll sorts it for you. I was born in Scotland and was told, by a solicitor, that a deed poll written up in England wouldn't have been worth the paper it was written on!:dance:Mash p'tater, mash p'tater:dance:0
