anyone cancelled with Vodafone?

Hi my girlfriend wants to cancel her contract does anyone have a template letter to send or something as never cancelled by letter before.

Also anyone know their address to send the cancellation too?

Well, if it helps I called them up, they asked me to send a letter & I wrote this (with my address at the top, of course):

Re: Cancellation of mobile telephone number XXXXX XXXXXX

Dear Sir or Madam,

As discussed in my earlier telephone conversation with your Cancellations Department, I would like to confirm my request to give one month’s notice to cancel the contract relating to the above mobile telephone number.
