Abbey/Santander mortgage PPI claim

Hi All,
I am new to the forum but have been trawling the post for the past couple of weeks and you have all finally given me the confidence to claim for PPI after reading your stories.
I plan to claim for 4 different PPI's.
1. For the first mortgage my husband and I had between 2005 and 2007 with Bank of Scotland. All I need to do is contact them and ask for the account number of the mortgage as I have no documentation at all. The reason I am applying against this mortgage is that I was in full time employment with NHS and would have received 6 months full pay and 6 months half pay in terms of sick pay and it was sold to us at the time that we would need additional cover. Does anyone know if they can reject a claim if the questionnaire is sent off without this account number?
2 & 3. Two policy's against our current mortgage with Santander (formerly Abbey National), one for life and one for critical illness. We are really unhappy about the way these were sold to us and subsequently cancelled after only 1 year of paying these. We were continually told during our meeting that we would insure our car so why not insure your life etc etc. Hence we felt obliged to buy the insurance. Would I have to send my questionnaire to Abbey who the mortgage was with originally or Santander? Could anyone help me out with an address for either of these? I have the forms completed and ready to send off, just struggling to find an address.
4. I had finance for my first car between 2004 and 2006 and I requested a copy of my agreement which they sent me. I noticed from the agreement that the total cost of credit was made up with the cost of the car plus "charges". I asked for clarification of the charges and they simply said interest and charges. This may be a long shot but I though about sending off a questionnaire to mini financial services and see how I get on. Should I complete a SAR form in order to get a more detailed breakdown of charges rather than corresponding via email?
