Plusnet avoiding Amazon voucher payout

Hello I wonder if anyone can help? I signed up for phone and broadband with Plusnet through and affiliate website for a promotion that included a discounted monthly fee and £30 of Amazon Vouchers, which never arrived. I asked Plusnet some months ago why I hadn't received them and was told there was a delay getting them out. Six month on from signing up I have asked for them again and now they want me to confirm which website I used when I applied for the deal. The problem is that I have no record of which site I used and they have said they cannot take this any further until I remember.
Can anyone suggest how I can take this further, such as using an independent official body who can put pressure on them to honour the agreement.

This appears to have been an offer that was clicked through from Broadbandchoices.co.uk, however was also posted on ispreview.co.uk and satellite-and-digital-tv.co.uk, is the first three that come back through Google.0

I don't see any agreement as you have no information (and neither does the other party as far as we know).
If someone promised me £30 I'd keep screengrabs of the offer, dates/times etc etc. In fact I do that everytime I use Quidco or any other site that offers me something for nothing.
