I can confirm that's right. I had the same problem and called them and they said it was the Dunstable address. I've since have confirmation they have received my cashback request.0

Sorry to scaremonger, but . . . . . .
Other posts on this newsgroup indicate that as the month in which you receive the phone (October in your case) is month 1 as far as e2Save are concerned, would this not make month 4 = January?

its says purchase october, Cash back due 4th month = January

so is mine i bought it on 24th nov, is this when u got yours? My first bill covered nov and dec so im guessing they count that as 2 months, so effectively we'r sending off our 5th bill which comes in the post?
also i got my auto cashback the other day without any phone calls etc!0 -
