New T-Mobile bills & Cool New Mobile Cashback

I had a contract with T-Mobile with a cashback deal with Cool New Mobile and everything has gone fine. I am now on my last cashback and T-Mobile have changed the layout of their bills. I sent in the first page of my bill, like I always do and I have just received a letter from Cool New Mobile (Mobile Affiliates) to say that I have not met the conditions of the offer.

It looks like I need to send in page 3 which shows my tariff details which were always on the first page!!!!!!!

I do not have anymore cashback forms to check the terms & conditions when I took the phone out a year ago, does anyone else have them?

This is soo annoying!!!
