First Direct/Telephone

I have received application pack for current account with £25 bonus but there was no second page which had to be signed & returned. Has this happened to anyone else - is it a delaying tactic or am I just being cynical?
Also I am with NTL telephone line & calls only - little usage evenings & W/E. Am I going to have to change to BT before I can then make use of other cheaper options or is there a way round this?

RE: First Direct - that's probably just a simple mistake; give them a call and explain, you will find them *very* helpful
