Equine Insurance - Claim Declined, HELP

Can anybody point me to where I might find sample letters to appeal against a declined insurance claim?
After weeks/months of trying to contact Insurance providers E&L, I eventually succeeded only to be told that our claim had been disputed and that a letter/email, had been sent over 2 months ago outlining the reasons for their decision. I have not received either one or the other.
"The found they were not liable because: said horse was being used in Type B activities whereby we only hold Type A Insurance cover". Our horse has never been used in Type B activities e.g. Novice Eventing only Type A, e.g. showjumping, dressage (unafilliated stuff).
Further more, as E&L have declined our claim on the grounds that the horse is being used as "Type B", is it acceptable that they have continued to take direct debit payments for "Type A" insurance?
I strongly believe that E&L's decision to decline our equine insurance claim is unfounded and therefore I wish to make an appeal. I am finding if difficult to compose a sraight forward appeal letter as my emotions are getting the best of me.
Also, I now want to change insurers but do not know what to do because I am afraid that if I cancel with E&L it will affect the outcome of my appeal.
Please can somebody help cos I'm feeling pretty miserable and desperate at the moment, I so hope this makes sense.

Good old E&L , they wriggled out of paying our Horse's vet bill a few years ago

I would maybe write back with a polite but firm email asking them what reason they have to believe your horse is being used as "Type B". I wouldn't ask why they were taking payment for "Type A" really as it would be your responsibility to ensure you have the correct insurance - for example, if you insured your horse as male by accident and it was actually female, you wouldn't expect the insurance company to have noticed and alerted you/changed your insurance premiums because of it - you'll probably find the smallprint specifies that you're responsible for providing accurate information.
Afraid I've heard nothing but bad of them too, but as they've given you a specific reason to reject the claim I would expect them to provide proof of it.0 -
You need to send them a letter titled "Official Complaint" and send it via recorded delivery, they have to properly investigate your complaint and come back to you within a set amount of weeks.
Keep the letter shortish and concise.
If possible get a letter on headed paper from the organisation who organised the event you were participating in confirming what type of event it was.0 -
Me and the vet made it absolutely clear that the injury to the horse happened in his stable. He wasn't even competing. Work that one out?
They sent one letter to us asking for additional information which included "type of activities the horse was involved in". Husband told them over the phone that it was "novice type events" eg unaffiliated show jumping, dressage. The adviser who took the call obviously thought he meant "Novice Eventing" which was not the case. Therein lies the problem.0 -

Changing insurer won't affect your appeal.
Considering paying more premiums to this outfit is just sending good money after bad!0
