I suggest you send the lead passenger a polite letter stating that you paid the money (give amount and date) so that she could book the holiday for you; she has now removed you from the booking so that you are unable to take the holiday; and so you would like the money back. Have you already offered to pay the cost of the name change? If so, mention that in the letter. If not, perhaps you could offer to pay half the fee for the name change (that way, if she pleads poverty or gets funny, you could agree to pay the whole of the name change fee so long as she gives you what is yours without any hassle).

Should she fail to respond to that letter, you should write again, politely stating that unless you receive the money due to you within seven days you will be forced to commence court proceedings. If that doesn't work, go along to your nearest county court (or visit the web site for MoneyClaimOnLine), fill in the forms, and add the court fee to the amount she owes you. I am confident that the court would find in your favour and order her to make payment to you.
