DLA form - statement from someone who knows you?

i have been used as a 'person who knows me' for several people in a professional context
and yes i have been contacted about the persons needs by phone
for clarification when what they put on the form sounded exaggerated to the decision maker.
i was able to reassure them as to the persons needs and they did get dla.0 -

I think there's a difference though when you're a professional giving an opinion as to family and friends.Lost my soulmate so life is empty.

I didn't bother putting anything in this section of the form. Seemed no point as they had my GP and consultant details and only my husband could fill it in, with the same details as I has already added.0

You can use more than one person by use of extra sheets. I had a friend who could say some things that they often help me with and observe. I hap a physio letter and supporting statement from hospital. I think anything you send is valid and different views and statements as long as contradicting just go to help build the big picture.0

no he wouldnt be called to give evidence, altho he couldaccompany you to a tribunial if there was one if you wanted him to.
chances are they wont even ring him to speak to him, i put a guy who worked with me at the time, they never contacted him at all.
i have had a tribunial and its not public speaking kinda thing, theres two or three at most folk from dwp and you sat at a table, theyl even give you a drink as it starts its no biggy. you are just having a conversation with them, you dont have to just sit and answer questions or call them sir or anything.0 -
Belatedly chipping in on the subject of supporting statements. When I made my last application, I enclosed about a dozen supporting statements from different people who knew me from different aspects of my life, including carers, relatives, friends, people who knew me through work etc. I think that building up an overall picture helped.0

having to fill in gf dla form for renewal in jan and i was gonna ask this very same question cause the person she usually uses is away for two weeks past form return deadline

My mum did my daughters DLA supporting statement, I enclosed no supporting statements and she got awarded higher rate care and lower rate mobility for an undiagnosed problem and that was only last week. This is much better than I expected.March prizes: 6 x Crufts tickets, tickets to Wham live! Heck naked sausages, DVD
