I did it all online. You have to register one of the codes on the back of the xbox (cant remember which one) and then fill in all the details. I got a reply very quickly confirming the repair would be covered by the warranty despite being outside of the warranty period.
The link retepetsir gave seems to be the one to use. The one I used was different but it has probably changed since then.

Have you tried contacting microsoft? Mine went a while ago and I knew it was out of warranty and also could not find the receipt but tried their online form. They paid for it to be collected, fixed it within a week for free and returned with a free 3 months xbox live gold card!
Worth a try if not already done so.
They collected and within a week,we both had a brand new Xbox each.0 -
I know when i sent mine back last time they sent me out a brand new machine, but that was a second repair & well, more than a little trouble getting to that stage including them telling me they'd repair it free then phoning me up to charge me, but that was 2 years ago and i've never had a problem since.Retired member - fed up with the general tone of the place.0

Yes, thinking back I think mine was actually replaced with a new one rather than being repaired as it had a quieter fan and a problem where the DVD drive didnt always read the disk was gone. Unfortunately I hadnt noted any of the serial numbers so cannot know for sure and my xbox was still unmarked.0
