Edited to add - the reason I no longer have it is due to my email system eating all mails prior to December. I have emailed them to ask for a duplicate form but I'm not holding my breath. If anyone who has it could PM me I'd be more than grateful!
Also - another question - it says to send it 120 days after connection, which in my case is the 28th of this month. Given that my bills are dated the 1st, do I wait for the next bill, or send the last one that I received. I know that you have to be very precise and I don't want to get it wrong. I would ideally like to send it recorded delivery this Friday I think. Anyone got any advice?
From the Ts and Cs
"Instalment 1: - Send us the claim form and original monthly bill showing your name, address, mobile number and tariff you are connected on, that arrives showing you have been connected for 4 months. i.e. if you were connected on the 1st of January, you need to send in the bill that is dated around the 1st of May. This is 120 days (4 months) after connection. See porting below "

"Where a valid claim has not been received by us within one calendar month of the required bill, which is the closing date for your claim. You will not be entitled to any part of the Cash Back offered.
