I had pretty much the same thing happen to me.Had new suspension fitted and the flexible brake lines weren't reattached securely to the struts so when I braked they moved. It resulted in one rubbing on the inside of the wheel and made a hole.I found out at 60-70mph approaching a roundabout. My brake pedal went to the floor and although there was some braking it was never going to be enough so I used the handbrake and after a bit of a tank slapper ended up going sideways down the dual carriageway but stopped without hitting anything.

I must have missed that bit. All I saw was "twisted 270" which is obvious to a monkey with a spanner that's just blindly tightened one end up without holding it properly, then thought, "sod it, it's my break now".
