    Go to https://www.dvla.gov.uk/dvla/onlineservices/order_forms.aspx?ext=dg and order the D2 pack. That is the pack required for 'vocational' licences.

    Fill the D2 form in. Tick all the provisional categories (D1, D, C1, C) because it does no harm, and may just save you from applying again in the future should you decide you want to learn to drive a full-sized bus or something.

    Get the D4 medical form filled in by a doctor. Your GP will probably charge quite a lot, but I had it done by driversmedicals.com for £52.

    Send both forms off together, preferrably recorded delivery. My new licence came back in 6 days with a shedload of provisional categories. Then you can book and do your theory test(s) and hazard perception test(s), and then book training afterwards. Good luck.

    Give Driver Training Wales a call. They can give you all the advice you need. A few of my friends and relatives have done courses with them and highly recommend them. HTH
    Still looking for the plot...... Anyone seen it???
