Long Term Fly-Drive to US

Hi There,
I'm due to travel to the US (Boston Area) with my wife and kids. We'll be there for nearly 3 months (the limit being 88 days + 2 days for travel) on a US Tourist Visa. I've got a couple of questions that I hope you guys will be able to help address.
FlyDrive:
I've been scouring all the usual suspects (Expedia, NetFlights, Virgin Holidays etc) for affordable Fly-Drive deals but keep on having a recurring problem that means they don't offer fly-drive deals for more than 26 days at a time. The cost of a car alone for the period will be around £2500 and I'm hoping to be able to combine the two.
Car Insurance:
Considering getting separate car insurance and skip the ones offered by the car hire companies. Is this recommended? Which vendors would you recommend in this regard. Also, if I hired two cars at the same time, would a single policy cover me for this?
Car Seats:
Most car rental companies charge $12+ per day for a car seat. I suppose, based on the length of stay, my best option would be to buy 2 new seats (one for each of the kids) from ToysRUs?
Travel Insurance:
I've already got travel insurance as an add-on benefit to other services I use. However this does not cover me for an extended period in the US. I've been looking into Annual Prestige from AMEX. Has anyone had experience in dealing with them and had any issues?
Any suggestions on the above would be greatly appreciated.
