DLA claim form 'Statement from somebody that knows you' query?

Options
I am preparimg to complete a DLA claim and have just read the form through. I notice this question (as my heading) that it is not mandatory but wondered if it is better to find someone, or if it makes little difference if I don't?

My problem is my illness/illnesses have cut my circle of friends right down to an absolute minimum. And (even though I am a man) I don't overly moan to them about how the problems have such a major effect on my life.

I also wonder if I can put my wife as she sure knows all my problems in full - eek!

Any help would be glady received and thanked.

I put down my friend/housemate as they've seen me when i've been ill and could give a good account of it.

What about using a professional you are in contact with, for example your GP?

We completed our son's and got his teacher to write a letter which we added.

I would be worried about using my GP as once he has filled in that section in you are stuck with it!!! I have been increasingly realising that I have never moaned to him enough about how my health affects my day to day life.

You are allowed to copy this page and take it to be filled in. If you are not happy with the report once completed, you don't have to use it-you can simply get someone else to fill in another page. Just make sure you fasten it to the rest of the pages when you submit.I copied it and took it down for the SENCO to fill out for my son the first time, the second time his class teacher completed it as she knew him best. I would have had no hesitation in leaving it out if I felt it didn't reflect his problems. If you took it in to your GP, then he may well ask you what he needs to put-my GP is very helpful like that, although I know not all are. I would definitely make an appointment and discuss your problems with him, as you are asked for your GP contact details in any case, and they can and do contact your GP (not always) and if you haven't discussed your current needs with him, then he may well fill out his report with 'no care needs known' which might mean an immediate refusal of your claim.
Tomorrow is always fresh, with no mistakes in it!

Hi SuziQ and thanks for the excellent advice.

I feel the GP's notes (if they truly reflect my condition) would carry more weight so very good tips. I really need to eductae him as to my condition and ask if he will give his full support.
