ESA query - what do I do????

I have been off work for a while, I am a nursery nurse (in special school on a casual basis which is usually every weeks I'm in for a few hours) and claimed ESA coz my earnings weren't high enough for SSP
I have been diagnosed with spondylosis - my neck vertebrae are crumbly and its trapping a nerve that despite lots of pills causes great pain all down my right arm/hand and Im right handed
The meds are making me dozy and I'm not on the full amount yet of nortriptaline and acupan so excuse any mistakes
If I continue to claim ESA will I be told to look for a different job? ie is this not considered bad enough. I did get a docs note that said ok to try work with no heavy lifting but employer didn't offer work
any suggestions - at almost 50 with a knackered neck I don't think my chances of nursery nursing are very high:(
