DVLA - Changed back to my maiden name, plus lost license..what do I do?

I have recently changed back to my maiden name (have legal documents)..I also lost my license and need to update it from the DVLA..will they allow me to change my name and give me a new license without supplying the old one?

Make sure before you send anything to dvla you photocopy EVERY bit of paperwork in case you disappear off their system

It has been known to happen !!
