DVLA - Changed back to my maiden name, plus lost license..what do I do?

they dont know if I lost it, Im going to tell them on the form, I'm going to check 3 boxes on the application:
Change of name
Change of address
Lost
Isn't this going to seem suspicious? Like I'm trying to create a new identity? lol..I do have all the legal documents though

Send the correct forms with the correct fee for a duplicate licence plus the legal documents and it wont be a problem, quote your licence number if you know it, it will be on the endorsement part of your licence unless that's also lost.
When I worked at the DVLA (13 years ago) we used to get applications for change of name and address after sex changes etc.
Its very common to have a change of name and address at the same time time due to marriage or divorce and many ppl lose documents during a move. What would cause suspicion is if wrong details were given, names spelt slightly wrong etc0 -
As long as you have the counterpart cant see a problem, don't forget if you don't send both parts of the licence back for a name/address change you have to pay!0

thanks mark, yes I have the green counterpart, should I send that in?
@gene, no it has been lost for around 2 weeks, after I moved to a new flat.
I would include the counterpart maybe with a post it note stating the photocard is lost.
I think the application form is a D1
Give your d.o.b, licence number, new name/title, address etc in the current details section and give your previous name/title/ address in the "to small" previous details box
If its been a while since your photo was renewed you might want to change this at the same time?
I cant remember what id would be needed in this instance.
Its been a long time since I left the DVLA so things may have changed a bit but we were each doing something like 200 licence applications a day, providing the forms, money and id etc were in order we would just process the licence and get on with the next one. A certain percentage might have been picked out for extra checks for security/fraud purposes but cant remember how many and it was carried out by more senior staff.0 -
