Who/what/where is ISA Manager of Halifax ISAs?

I'm transferring a matured cash ISA from Halifax, and the transfer form of the new chaps requires the "Name/address of current ISA manager". Does anyone know what to write here? I could call Halifax, but I expect I'd have to endure cross-selling and/or attempts to get me to reinvest.
The Halifax website is not as helpful as I might like, and I'm loathe to just write "hALiFax" in badly scrawled crayon.

Technically Bank of Scotland plc trading as Halifax, but Halifax has always worked perfectly for me. As dunstonh says, use the address from your statements.0
