Put your reasons for leaving in writing directly to HR before you leave and keep a copy in case you need it in future.

Thanks but should I put it formaly ot informally? Formally mean HR has to investigate the matter whereas informally mean it will stay on his file for future and only investigated if he makes another dishonesty.

I dont mind even if I dont get JSA but problem is that my manager is willing to backstab me even at my new job and has hinted that he will go to any length.
Thus to protect myself from his malice in future I need to make a formal/informal complaint so I may mention it in future.
The only thing I have now is my grievances comlaint emailed to him.

Inform him that if he harasses you at your new job, then you will make a formal complaint about him to the Police. Sounds like a bully who just needs to be put in his place.
