Cancelling Virgin Media Broadband query

Hi, would be grateful if someone with experience could help.

My Virgin Media Broadband (Cable) 12 months contract ends at beginning of September. Looking at their T&Cs, it kind of suggests that you can only give 30 days notice after the minimum contract term.

Does that mean I cannot give notice until 12 months have completed and therefore disconnect at the end of month 13?

Most 12 months contracts on mobile phones or Sky, you are allowed to give the notice at month 11.

Have I understood their cancellation terms correctly?
Also, do I need to return them both the modem and wireless router?

Not sure if it is just the modem that forms part of their hire-equipment and the Netgear wireless router classed as a 'freebie'.
