Regarding the "would you re-employ", I worked for a multi-national where the rule was that, unless perhaps if someone chose to leave because they became a mother, the company would always say "would not re-employ" - because someone resigning meant they were not happy with the company and therefore it would not be appropriate for them to return. It did not mean that the person was not a perfect employee.

Why would you leave a company if you were not in some way dis-satisfied with your employment there? If it is to go for a better job, higher pay, more holidays, a better car or whatever then you are not satisfied with the company.
(By the way, they did actually sometimes re-employ people though.)

I'd look at what the reference is asking for. If it is just dates and a wage then give the facts, including the notice period given.QUOTE]
It asks for absences (quite a few) timekeeping (ok) conduct (ok) notice given (24hrs) and a box for `would you employ this person again` (no)
Write exactly that. (well except I'd put the number of days absent over what period instead but thats obvious).
They will draw their own conclusions and you are not responsible for her prior behavior!0 -

sometimes companies give you no choice and will not proceed unless you will agree to them contacting your last employer.
When I had that problem due to an illness related issue I clued the recruiter in on the issue before they went for the reference so they knew what was likely to be reflected in the reference.0 -
