Sounds like you can truthfully answer their questions and ensure she doesn't get the job.
I hope she's out of time for taking you to an ET now, because if she's not, that might rile her enough to consider it again.
I'm not sure, but I believe you're supposed to be consistent with your reference policy. Even if it's not a legal requirement, it's generally a good idea to be consistent, so you might want to consider that in future you will only confirm dates of employement unless asked specific questions, which you will answer truthfully. Would you tell the OP then to lie on a referance form?

looking back on that he lost on a technicality on a risk assesment and not the main reason he was taken to an ET for.The Googlewhacker referance is to Dave Gorman and not to my opinion of the search engine!

Also I think it was a different person anyway.....if they are asking for a reference after 3 months then it's possibly because the OP wasn't their first choice and other people may have declined. Ergo; it might be better to just not respond and then nobody gets any grief.0

I thank you for your letter of xxxxx requesting a reference for Miss X. I can tell you that she was employed by my company from aa. to bb. Whilst she was in my employment, she carried out her contractual duties, however when she resigned from her position she did not comply with the contractual notice period.
I hope this information is sufficient for your files.0
