Do Virgin often c0ck things up?

We moved in the middle of March, and Virgin were meant to be coming to install at the beginning of March.
They cancelled on us and changed the date to one when we were already moved in, which was a pain.
Then we received the first bill. With £5 added to it as a non DD charge.
Rang them up, and told them there was a DD set up on the account, so they said OK, take £5 from the balance showing on the bill and that's what will come out of the bank in 14 (or so) days.
Got a letter from them confirming the DD was set up.
Just received an email with this months bill on it.
They didn't take last months DD and they have slapped a £10 late payment charge on it.
What are they playing at?!
I didn't know they hadn't took it as it comes from OH's bank account and we've had that much on these past few weeks that he didn't check that it had come out.
Do they normally balls things up all the time?
Also, one more question, why do they have their billing the way they do? ie, 3 x multi room = £15, so that's what you would expect to show on the bill, only it says £45 (???) and then a discount of £28odd, what's all that about?
