Abbey Mini-Cash ISA - Transfer paperwork problem!

Hi,
A family member is wanting to transfer their mini-cash ISA from the Abbey to a new provider. Its a branch-based ISA and I think its also a CAT standard. However, they're getting the runaround between new provider (NP) and Abbey as to who is responsible for generating the transfer paperwork - NP says its the Abbey and they're playing dumb
Is it the Abbey that should generate the paperwork?
If so, what should they explicitly ask the Abbey do viz the paperwork?

the new provider sends the paper work to the old provider and then sits back and waits for the funds.I am an Independent Financial Adviser (IFA). When the ISA was opened at Abbey the welcome pack should have contained an ISA transfer form.
The tel number is 0845 6000181 for Cust ServicesI beep for Robins - Beep Beep

An update .....
Having contacted the Abbey by phone they sent out a form which only allows you to transfer between Abbey ISAs.
Next, went to the main office in Glasgow only to be told that "we don't do transfer out forms". Abbey have taken the account number and apparently will contact the new provider.
Has anybody transferred out from an Abbey Easy ISA? And if so, is this the palaver you have to go through?0
