TSB PPI number?

When requesting a refund for PPI, do I need the insurance number, or just the loan number, as it was all done through TSB (loan and insurance)?
I sent TSB a letter asking for the documents for the loan that I took out, including all details for the PPI.
They got back to me within 5 days, with the paperwork, but doen't mention the PPI number, just the loan number, and the agreement for the PPI (which I believe was mis-sold, and they took advantage of me because of my age when taking the loan out in 2006)
Surely I could still go ahead without the insurance number and just the loan number as its all on the TSB system?

On the 07/07/2006 I was approved a loan for £6012.82. I was advised that as a condition of the loan, loan protection insurance (LPI) would have to be purched; as it would insure payments would covered if I were to become unemployed or was unable to work. This was in the form of an additional loan totaling £1425.89
It is fact that you are not allowed to make (LPI) a condition of a loan.
It was not something that I required, as I had savings accounts and also had backing from my parents if required to help cover repayments, but was told that if I wanted the loan, then I must have the LPI. I therefore consider that I was mis-sold this expensive insurance, and request that a full refund of all costs incurred through the aforementioned LPI, including all associated interest.
This loan was settled on 23/10/2008, and was paid off in full, including the LPI.
[FONT="] I would appreciate an immediate response to this letter that will conclude this matter as mentioned.
Do i need to add anything or leave something out?
and do i send them a copy of the questionaire that is on the link above?

Hi
Yes this is fine, although you do not have to write about the amounts if you don't want to, because if successful and they uphold in your favour they must also add interest of this and that, and you can also request they compensate you the 8% interest of what a court would award for being deprived of your money.
Also make them aware you have been out of pocket, you assumed the PPI was all part of the finance, if you were informed and if the policy was discussed with you - then you would have had the opportunity to inform them you would not require this because of the reasons you given as above.
Also mention that you don't believe it was sold in your best interest.
This would also make the letter stronger.
Overall a great letter, and if successful they should supply you with the detailed breakdown or you request they send you this, but keep at them.
As they have 8 weeks entitlement to reclaiming, tell them you expect to hear from them by the 8 weeks with a final decision, or you will consider taking further action with your case by requesting the Financial Ombudsman Service (FOS) to investigate, or to the courts.

However, you are doing the right thing by using your own words, in my experience I have been more successful directly with the bank by writing my own words and not using a template, although the template is useful just to make sure you don't miss anything, such as the FOS etc.

Made a few changes. should i also mention anything about statutory interest at 8% in my request?
do I need to send a completed FOS Questionnaire to them with the letter, or is this for the next stage, if it goes that far?

You can of course mention that interest if you want to, to make sure nothing is missed and to cover al grounds.
I take it the Questionnaire if the one to complete to the FOS alongside with the complaints form, if your not happy with the outcome of the decision, unless someone can clarify otherwise.
I understand there have been some changes with the FOS in regards of this new questionnaire, if a PPI complaint, there is questionnaire to complete for this matter and a complaints form to be completed too.
I am not aware of a questionnaire to sent to reclaim, but sometimes banks have their own that they send you if they require further information, they all vary.;)

....goes on to say they'll do their best to resolve my complaint within 8 weeks, and send me a letter in 4 weeks with a progress report (if not already resolved), then if they can't make a final decision within 8 weeks, they'll send me a letter explaining that i can then refer my case to the FOS
