Leaving BT at end of contract

I have BT Business Total Broadband for 2yrs ending May 2011. I want to leave BT at that point (to avoid paying for remainder of period anyway). Should be straightforward I know, but want to make sure I don't get caught out by auto-renewal or something. I also want to be sure of getting uninterrupted service at switchover as far as possible!
I have in mind to write BT about 3 months in advance, and arrange precise date with new provider. Any advice to ensure a smooth handover?
