After replacing the bath mixer tap this morning (the old one appears to have a dead washer and I can't access it - the head screw is rounded off) - I have a leak on the hot feed...
I have used PTFE tape (5+ layers) - but the leak is still there
Any suggestions ?

It seems to have a lot of DIY people there and they might have some advice for you.There is a race of men that don't fit in; A race that can't stand still;

If the leak is between the tap tail and the tap connector ensure the washer is in place, of the correct type and seated properly.
Make sure the tap connector nut is going on square and not cross threaded.
PTFE tape if necessary needs to be wrapped around the tap connector (on top of the washer) not around the threads of the tap tail. 5 layers is too much and more than you can properly wrap around the tap connector. Not usually required but 2 turns of tape should be enough.0 -
There was no washer in the kit0
