    This charge is between you and your doctor.
    You want a 'service' from your doctor that is not covered under the NHS.
    So he charges you for it.

    It's nothing at all to do with the insurance company - they are NOT adding further costs to your claim.
    They only want proof that your injury is sufficient to prevent you going on holiday.

    So don't rant at the insurance company, take it up with your doctor.
