    NO you mis-understand, I do not want to fill the forms in online, I want to DOWNLOAD the full form for completion off-line and to take with me to the court.

    Hence my use of the word 'download' and not 'online' ....

    Just download them and fill them out offline.

    Or go to your local Insolvency Service office and they'll give you a copy of the booklet.

    But do check that you are completing the "correct" form - the PIQ - B is not given to the court.
